ABSTRACT
Objective: The aim of this study was to evaluate the association of serum and urinary levels of the chemokine CCL5 with the stages of diabetic kidney disease in non-dialysis patients. Method: The study included 80 patients with type 2 diabetes mellitus attended at the nephrology outpatient clinics of a large hospital and the Polyclinic of Divinópolis, Minas Gerais. Albuminuria was measured by immunoturbidimetry, serum and urinary creatinine levels were determined using the modified Jaffé colorimetric method, and the urinary albumin-to-creatinine ratio (ACR) was calculated. The estimated glomerular filtration rate (eGFR) was calculated using the CKD-EPI equation. Serum and urinary CCL5 levels were determined by ELISA, and the urinary CCL5-to-creatinine ratio was calculated. A p-value < 0.05 was considered statistically significant. Results: Patients with ACR ≥ 30 mg/g showed a trend towards an increase in the urinary CCL5/creatinine ratio compared to those with ACR < 30 mg/g (p = 0.095). Patients with eGFR < 15 mL/min/1.73m² had higher serum CCL5 levels than those with eGFR between 15 and 29 mL/min/1.73m² (p = 0.010). Conclusion: The trend towards increased urinary CCL5/creatinine ratio in patients with elevated albuminuria suggests an association between local kidney inflammation and increased albuminuria. The elevated serum CCL5 levels in patients with end-stage chronic kidney disease indicate exacerbated systemic inflammation in these patients.
